Heel Pain in Children

Children often complain of heel pain, and it is usually not serious.  However, it is important to understand what is going on as such pain can arise for various reasons. If a child suffers from pain or tenderness in the back of the foot or ankle, limps, or walks on their toes, they might have Achilles tendonitis or Sever’s disease. Achilles tendonitis is an inflamed Achilles tendon. Sever’s disease, or calcaneal apophysitis, affects kids from five to eleven years of age and involves the Achilles tendon pulling on a growing heel bone. Both conditions can occur from overuse or a sudden increase in activity.  Children performing repetitive movement as in sports like soccer, basketball, and track that involve running or jumping are more prone to heel pain. Rest, ice, compression, elevation, stretching, over-the-counter pain medication, and perhaps cushioned heel lifts usually provide relief, but ignoring symptoms can lead to worsening of the injury and sometimes chronic pain. Other less common reasons for heel pain in children are plantar fasciitis, fractures of the heel or foot, infections, tumors, or congenital problems. If one suspects a child has sustained a fracture or if pain persists beyond a couple of weeks, a podiatrist should be seen to examine your child’s feet, determine what the cause of pain is, and suggest an appropriate treatment plan.

Causes of Heel Pain

Heel pain is often associated with plantar fasciitis. The plantar fascia is a band of tissues that extends along the bottom of the foot. A rip or tear in this ligament can cause inflammation of the tissue.

Achilles tendonitis is another cause of heel pain. Inflammation of the Achilles tendon will cause pain from fractures and muscle tearing. Lack of flexibility is also another symptom.

Heel spurs are another cause of pain. When the tissues of the plantar fascia undergo a great deal of stress, it can lead to ligament separation from the heel bone, causing heel spurs.

Heel Pain

Have you ever gotten up from a chair or out of bed in the morning, and upon taking that first step, feel like your heel has stepped on a tack? Many people experience a feeling of sharp pain which radiates into their arch from their heel and which does not allow them to put their heel on the floor. Sometimes they need to sit back down, stand only on their toes and use the wall for balance. If you can take a few steps, it seems to go away and lessen, allowing you to then resume your activity. Later, throughout your day and after a period of rest, it can happen again. If this sounds familiar you may be suffering from your first attack of heel pain.

Heel pain is a debilitating condition that affects day to day activities. Running and walking both causes stress on the heel because the heel is the part of the foot that hits the ground first. Plantar Fasciitis

One of the main causes of heel pain is a condition known as plantar fasciitis. The plantar fascia is a band of tissue that extends along the bottom of the foot, from the toe to the bottom of the heel. People who do not wear proper fitting shoes are often at risk of developing problems such as plantar fasciitis. Unnecessary stress from ill-fitting shoes, weight change, excessive running, and wearing non-supportive shoes on hard surfaces are all causes of plantar fasciitis.

Achilles Tendonitis

Achilles tendonitis is another cause of heel pain. Similar to plantar fasciitis, inflammation of the Achilles tendon will cause heel pain due to stress fractures and muscle tearing. A lack of flexibility of the ankle and heel is an indicator of Achilles tendonitis. If left untreated, this condition can lead to plantar fasciitis and cause even more pain on your heel.

Heel Spur

A third cause of heel pain is a heel spur. A heel spur occurs when the tissues of the plantar fascia undergo a great deal of stress, leading to a separation of the ligament from the heel bone entirely. This results in a pointed fragment of bone on the ball of the foot, known as a heel spur.

Ways to Prevent Falls in the Elderly

According to the CDC, one out of four seniors over 65 experiences a fall, and the odds are that once you have fallen, it will happen again. This often occurs as the result of diminishing balance, mobility and coordination as you age. Five simple ways to help prevent falls, and the injuries that may follow, include removing tripping hazards around the home, making stairs and bathrooms safer, wearing proper shoes and slippers, using a cane or walker, and doing exercises that both strengthen legs and feet and improve balance and coordination. First, it is suggested that you remove area rugs or mats that can be tripped over or that slide too easily, and clear hallways and walkways of clutter or other objects. Second, have railings installed on all stairways, including those on porches or home entry areas. Also, add grab bars to bathroom tubs and showers and make sure to use non-slip mats. Third, find shoes that provide support and fit well. Avoid going barefoot and walking around in socks at home. Instead get shoes and slippers with non-skid soles for safety both inside and out of the house. Fourth, it is amazing how many seniors refuse to use a walking aid out of vanity, but using a cane or walker is one of the best ways to avoid falls. Finally, see your podiatrist for information on how to strengthen your ankles and feet with exercises you can do at home.  A podiatrist can answer any questions you may have on ways to prevent falling.

Falls Prevention

Elderly Americans are very susceptible to falls as they get older. Everyone experiences decreases in flexibility, balance, strength, and the senses as they age. This correlates to some eye-opening statistics. 1 in 4 Americans aged 65 and older fall each year. An elderly American is being treated for a fall in an emergency room every 11 seconds. In light of these striking statistics, one can see the importance of taking steps to prevent falls.

Finding an exercise program for the elderly is an excellent way to reduce the likelihood of falls. Look for an exercise program that improves strength and balance. Elderly people who live a more sedentary lifestyle, with little physical activity, are at an increased risk of falling. Wearing well-fitted footwear that provides good foot support and cushion will help prevent falls from poorly fitted shoes. Talking to a podiatrist about your susceptibility to falls and about inspecting your prescriptions will help to avoid any medication that could make falls more likely. Due to a decline in the senses among the elderly, having your eyes and hearing checked is recommended.

Around half of all falls occur in the household. Removing tripping hazards in the home and making it more accommodating to older persons can significantly reduce falls. Gout

Gout is a form of arthritis that is caused by a buildup of uric acid crystals in the joints. This considered to be one of the most frequently recorded medical illnesses throughout history. Gout occurrences in the US have risen within the past twenty years and the condition now affects 8.3 million people which is 4% of all Americans. Researchers have found that gout affects men more than women and African-American men more than white men. 

Symptoms of gout are warmth, swelling, discoloration, and tenderness in the affected joint area. The small joint on the big toe is the most common place for a gout attack to occur.

People who are obese, gain weight excessively, drink alcohol heavily, have high blood pressure, or have abnormal kidney function are more likely to develop gout. Furthermore, certain drugs and diseases are likely to increase levels of uric acid in the joints which eventually leads to gout. You are also more likely to develop gout if you eat a lot of meat and fish.

Many who experience gout attacks will experience repeated attacks over the years. Some people who have gout symptoms, may never have them again, but others may experience them several times a year. If you have gout symptoms throughout the year, you may have recurrent gout. Those who have gout should also be careful about their urate crystals collecting in their urinary tract, because this may lead to kidney stones.

Diagnosis for gout is done by checking the level of uric acid in the joints and blood. Your podiatrist may also prescribe medicine to reduce uric acid buildup in the blood, which will help prevent any gout attacks.

To treat gout, your podiatrist may also prescribe you Anti-inflammatory medication (NSAIDs) which will relieve the pain and swelling of a gout episode and it can also shorten a gout attack. Maintaining a healthy diet is also a proven method to prevent gout attacks. 

Your feet are covered most of the day. If you're diabetic, periodic screening is important for good health. Numbness is often a sign of diabetic foot and can mask a sore or wound.

Understanding an Achilles Tendon Rupture

An Achilles tendon rupture is an injury that happens when there is sudden and overwhelming pressure put on the Achilles tendon – the band of fibrous tissue that links the muscles of the calf to the heel. There will usually be a popping noise and intense pain with this injury and the area may burn and become swollen and stiff. You will have trouble standing and pushing off from that leg. Rest, cold compresses, physical therapy, and attention to a proper diet can all help, but this condition is slow to heal. A podiatrist can perform what is known as the Thompson Test to determine if you have ruptured your Achilles tendon. This test consists of a simple manipulation of the back of the calf, specifically, your soleus complex muscle group. If the area does not tense up and bend (a natural reflex of the manipulation), you may have ruptured this major tendon or there has been a detachment of the tendon from the muscle or bone. A consultation with a podiatrist is suggested if you suspect you have injured or ruptured your Achilles tendon for a proper treatment plan.
